How can I schedule an evaluation for my child? (Insurance Based Services)
- Contact us to confirm we accept your insurance.
- Fill out the information under “contact us” we will then send you an intake form to complete.
- You will need to request a referral from your child’s pediatrician be sent to Elevate Therapy Center via fax, or you may drop it off in person.
- Once the intake forms and referral are received, we will request authorization from your insurance company. This process can take from days to weeks and varies depending on the insurance.
- Once we have received intake forms, referral, and insurance authorization, we will contact you to schedule your child’s evaluation.
How can I schedule an evaluation for my child? (Private Pay Services)
- Fill out the information under “contact us” we will then send you an intake form to complete.
- A referral is not needed to schedule an evaluation, once we have received your intake form, we will contact you to schedule your evaluation.

How long will my child need therapy for?
Every child is unique and has a different diagnosis or severity. During the initial evaluation the therapist will determine the frequency (sessions per week) and duration (in months) for your child’s therapy.
What happens if I miss or cancel an appointment?
Consistency is imperative for your child’s growth and progress. Please note that your child’s appointment is reserved solely for him/her, and much preparation goes into each session. If frequent no-shows and cancellations are made, your child may be removed from our therapist’s schedule. This allows the opportunity for another child to receive therapy.
Is Elevate Speech Therapy licensed?
Yes, we are licensed by ASHA, the American Speech-Language-Hearing Association, to provide speech therapy services in Florida.
